• Athletic Packet Information

    In order for a student to participate in athletics at Maize South Middle School students must have their athletic packet filled out in Skyward.

    1. To find the athletic packet you must login to family access in Skyward 

     

    1. Next on the left hand side you find the custom forms. 

     

    1. Under Custom Forms you will see one that says athletic packet. Click on the edit button. 

     

    1. You must read and sign everything listed in the athletic packet. If you miss something it will not let you go any further. 

     

    1. Once you have completed the athletic packet click on submit.
