This ESI Resource includes further definitions and descriptions, as well as information about when ESI may be used, related requirements, parent notifications, and how to file a complaint.
Parents have a right to file a complaint through the local dispute resolution process and the Kansas State Board of Education’s (KSBE) administrative review process. ESI Concerns
If you have concerns, you have options when the school uses restraint or seclusion on your child. There are four ways to voice your concerns:
- Request to hold a meeting with the school about the ESI.
- Request a meeting with the principal.
- File a written complaint with the local school board within 30 days after you were told about the incident.
- If you do not get a decision from the local school board within 30 days or you do not agree with the decision of the local school board’s investigation, request an administrative review by the state board of education.

FERPA
2024-25 Maize USD 266 Family Educational Rights and Privacy Act (FERPA) Enrollment Form
Families can find the FERPA enrollment form in Skyward Family Access. The form includes the following information.
The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that Maize USD 266, with certain exceptions, obtain your consent prior to the disclosure of personally identifiable information from your child's education records. However, Maize USD 266 may disclose appropriately designated “directory information” without consent, unless you have advised Maize USD 266 to the contrary in accordance with district procedures. The primary purpose of directory information is to allow Maize USD 266 to include this type of information from your child's education records in certain communication tools. It is necessary for a parent/guardian to advise the district if you do not wish to give consent for “directory information” items. “Directory information” may be used for, but is not limited to:
- District and school e-newsletters, newspapers, news releases to local and area media
- District and school websites, blogs, and official social media sites
- General district marketing materials, publications, and mailings
- Honor roll or other recognition lists
- Yearbook
- Parent-Teacher Organization (PTO) promotions and communications
According to federal guidelines, directory information, which is information that is generally not considered harmful or an invasion of privacy if released, may be disclosed without a parent's prior consent. Outside organizations requesting directory information include, but are not limited to, companies that manufacture class rings or publish yearbooks and PTOs. In addition, two federal laws require local educational agencies (LEAs) receiving assistance under the Elementary and Secondary Education Act of 1965 (ESEA) to provide military recruiters, upon request, with three directory information categories – names, address and telephone listings – unless parents have advised the LEA that they do not want their child's information disclosed without their prior consent.
Maize USD 266 has designated the following information as directory information:
- Student name
- Parent name(s)
- Address
- Telephone listing
- Parent email address
- Photographs
- Video images
- Audio recordings
- Date of birth
- Dates of attendance
- Grade level
- Participation in officially recognized activities and sports
- Weight and height of members of athletic teams
- Degrees, honors, and awards received
- The most recent education agency or institution attended
If you do not want Maize USD 266 to disclose directory information as outlined above, you must fill out the FERPA enrollment form in Skyward Family Access.
*These laws are: Family Educational Rights and Privacy Act, 20 U.S.C. 1232g, and section 9528 of the ESEA (20 U.S.C 7908), as amended by the No Child Left Behind Act of 2001 (P.L. 107), and 10 U.S.C. 503, as amended by section 544, the National Defense Authorization Act for Fiscal Year 2002 (P.L. 107-107), the legislation that provides funding for the Nation's armed forces.
